Post-Operative Instructions: Permanent Nail Procedure (Matrixectomy)

You have undergone a chemical matrixectomy today to treat an ingrown or damaged toenail. This involves removal of part or all of the nail and use of a chemical (typically phenol) to prevent regrowth.

Please follow the instructions below carefully to promote healing and reduce the risk of infection:

🔸 1. Wound Care (Days 1–3)

  • Leave the bandage on for 24 hours. Keep the area clean and dry.
     
  • After 24 hours, begin daily warm Epsom salt soaks:
     
    • Use lukewarm water and about 1 tablespoon of Epsom salt
       
    • Soak the affected toe for 15–20 minutes
       
    • Pat dry with a clean towel or gauze
       

🔸 2. Topical Care

  • After each soak, apply a small amount of antibiotic ointment or Mupirocin (if prescribed) directly to the wound.
     
  • Cover lightly with a Band-Aid or sterile dressing.
     
  • Repeat daily soaks and dressing changes for at least 10–14 days or as directed.
     

🔸 3. Activity

  • Limit excessive walking or pressure on the foot for the first few days.
     
  • Wear open-toe shoes, sandals, or loose-fitting shoes to avoid irritation.
     
  • Avoid tight socks or shoes that put pressure on the surgical site.
     

🔸 4. Pain Management

  • Some mild discomfort is expected for 1–2 days.
     
  • You may take Tylenol or ibuprofen as needed unless otherwise directed and if not contraindicated by certain other disease states.  For example if you are on a blood thinner then taking ibuprofen with that is not acceptable. Also, if you have a history of stomach irritation especially ulcers is definitely not recommended for use with ibuprofen.
     
  • Avoid aspirin unless prescribed, as it may increase bleeding.
     

🔸 5. Watch For Signs of Infection

Please call us immediately if you notice:

  • Increasing redness, swelling, or warmth
     
  • Pus or yellow-green drainage
     
  • Fever or chills
     
  • Severe or worsening pain after the first 48 hours
     

🔸 6. Follow-Up Appointment

  • Your follow-up visit is scheduled in 2 weeks. If you don’t have one scheduled, please call our office.
